Where do action items come from?

How does escalation work?

Each severity level has a response target. When an action approaches or passes it, the owner and their manager are notified automatically, and it is flagged on the dashboard.

Can the same person raise, complete and verify an action?

No. Raising, completing and verifying are kept separate so that closure always includes an independent check.
